(1) A bipolar CS lead (Medtronic2188) positioned in the proximal part
of the great cardiac vein to pace the LV anterior base
(2) a unipolar convential lead inserted in an anterolateraal vein over
the LV free wall
(3) the right ventricular lead is placed at the RV apex.
